CSRS collects information through our Website at several points. We collect the following information about primary visitors: name, address, phone number and email address. (The CSRS does not actively market to children and we never knowingly ask a child under 13 years of age to divulge personal information.) We collect this information through registration forms. The information is collected voluntarily. To receive our products and services, name, address, and phone number and/or email address is necessary so that referrals to Board Certified Plastic Surgeons can be tracked.
Part II. Information Usage
The information collected by the CSRS will be used for referrals only. Users who provide information will receive referral services. Registered users will receive additional announcements from us about products, services, special offers and a newsletter. Out of respect for our users we present the option to not receive these types of communications. The information we collect will not be used to create customer profiles based on browsing or purchasing history. We will supplement information collected at our Website with data from other sources but only when the patient requests financial assistance. Upon approval from the patient we may share data with a third party for the sole purpose of acquiring a loan. This third party will use the information as follows: to determine whether or not financing can be approved for the patient requesting financing.

Part III. Access to Information
The CSRS maintains the accuracy of our information by monitoring daily. Users may access their own personal information and contact us about inaccuracies they may find by calling on the telephone, faxing, email or in writing. Users may delete their information from our database by contacting the Association office at 1-866-367-9310.
